Due to a new state law passed this year, no school shall allow a student under the age of 18 to become a member of a club or organization without the parent or guardian's consent. Therefore, all Houston Middle School parents/guardians wishing for their child to join a club or organization must complete this form prior to their student participating.

Please note that this form is solely for parental consent and does not imply student enrollment or assure membership or induction into any club or organization.
